Opening of Mountain Barangay Gravel Roads

(This is applicable only in Cebu, Philippines.)

PROCEDURES ON HOW TO REQUEST ROAD MAINTENANCE IN CEBU:

Follow these steps...	Wait while Person-in-charge...	Approach...	It will take...
1. Submit request letter to DEPW Office	1. Reviews request letter and attachment	Aniceta Perez, Receiving Clerk	30 minutes
	2. Forwards request letter to City Engineer for assignment to concerned Sectin Head.	Aniceta Perez, Receiving Clerk
	3. Assigns capataz for inspection	Apollo Dante, Section Head	10 minutes
2. Follow up request for schedule of repair/asphalting	4. Coordinates with dient for schedule	Raul L. Durangparang	8 hours
3. On the scheduled date, be at the designated area to guide the Maintenance Personnel.	5. Conducts repair or asphalting.	Capataz and laborers	8 hours
	6. Gives assignment sheet for client's signature.	Raul L. Durangparang
4. Sign assignment sheet after the work is completed.		Raul L. Durangparang
