In paying their premiums, payors simply need to accomplish the PhilHealth Premium Payment Order Slip (PPOS) indicating the following: * Date of payment
* PhilHealth Identification Number / PhilHealth Employer Number
* Complete Business / Agency / Member’s Name
* Member type
* Applicable period
* Amount of premium to be paid
SM Retail, Inc. shall issue a Point-of-Sale (POS)-generated receipt (PGR) and shall validate the PPOS and both shall be recognized as proof of PhilHealth premium payments.
