How do you convert AVI to FLV using Avidemux in Windows?

How do you convert AVI to FLV using Avidemux in Windows? I've tried the tutorial but there seems to be a specific file that has to be associated with Avidemux. I'm using version 2.5.1 (r5249)

"I've just converted a video to FLV using Avidemux 2.5.2 on Ubuntu. While I realize that your question is about Avidemux on Windows, I think that they should work pretty similarly on both platforms. The key things were:
