Join two h.264 movie files, merging chapters?

Join two h.264 movie files, merging chapters? I have two h.264 encoded .m4v files, generated with Handbrake 0.9.3. They used the same Handbrake preset. How can I merge the two files into one movie, and do I need to do anything special to preserve chapter marks?
