What is causing the attachment head to start falling off during use.

What is causing the attachment head to start falling off during use. The part that the attachments connect to has started slipping. Managed to get it back into place, and it is falling down again. Can this be a simple gear switch out? Looked at troubleshooting, yet still not sure which one pertains to my issue.
