REQUIREMENTS:
- 3 copies of recent 2x2 picture (colored or black and white)
- Chest X-ray result with big film
- Residence certificate
- Birth Certificate (for 18 to 25 years old)
- Php 25.00 receipt:
- Php 10.00 for Health Card
- Php 15.00 for Gram staining
