This service caters any petition from concerned clients for a correction of entry or change of first name of civil documents through R.A. 9048.

AVAILABILITY :  Monday to Friday, 8:00 AM - 5: 00 PM

DURATION OF THE SERVICE:  17 days & 20 minutes

REQUIREMENT/S:
  1. Petition for CCE
    • Accomplished Petition Form RA9048 1.1(CCE)
    • If petitioner is not document owner or parents then attach Power-of-Attorney;
    • Certificate of Birth (NSO copy)
    • Certificate of Birth (Local copy)
    • At least two (2) of the following:
      • Baptismal Certificate
      • COMELEC Voter’s record
      • GSIS or SSS record
      • Driver’s License
      • Land Title/Certificate Transfer of Title
      • School Record
      • Bank Records
      • Others (as required by the Civil Registrar)
    • Required Clearances, to wit:
      • Certificate of Employment with no pending administrative case (for employed) or Affidavit of Unemployment (for unemployed applicants);  Business permit (self-employed)
      • NBI Clearance
      • PNP Clearance
  2. Petition for Change of Name
    • Accomplished Petition form RA9048 4.1 (CFN)
    • If Petitioner is not the document owner or parents then attach Power-of-Attorney;
    • Certificate of Birth (NSO copy)
    • Certificate of Birth (Local copy)
    • At least two (2) of the following public or private documents upon which correction shall be based, to wit:
      • Baptismal Certificate
      • COMELEC Voter’s Affidavit/record
      • Employment Record
      • GSIS or SSS record
      • Medical Record
      • Business Record
      • Driver’s License
      • Land Title/Certificate of Land Transfer
      • Insurance Record
      • Bank passbook
      • Others (as required by the Civil Registrar)

Dear Atty. Evangeline T. Abatayo

I have a problem with the spelling of my middle name as appearing in my birth certificate. The correct spelling of my middle name is “Menguito” but it was mistyped in my birth certificate as “Mingueto”. I already filed with the office of the Local Civil Registry of Cebu City my application for correction of my name, but until now I have not received any feedback as to whether they granted my application or not. I just want to know whether I have other options in correcting the error in my middle name.
