Hood has gaps at the rear close to windshield

Hood has gaps at the rear close to windshield I have 2001 dodge ram 1500 5.9 my hood has huge gap on both side near windshield . Also hood looks like it slid forward it's not lined up with fender sides. How can I fix Xbox this problem.
