REQUIREMENTS:
- For new clients:
- forms available in the admission area; to be filled up by a Staff based on your information given
- For Previous clients:
- Home Based Maternal Record
- Early Childhood Care and Development Card (ECCD)
- Vitamin A Supplements
- Fees / Charges - none

| 1. Get priority number at the admission area. | 1. Instructs the client to wait for the number to be called. | Barangay Health Worker or BHW | 30 seconds |
| 2. Hand in your record to the staff in charge | 2. Locates the record in the shelf and give it to the staff | BHW | 5 minutes |
| 3. Wait for your turn for the weighing and blood Pressure taking. | 3. Calls the number, weigh, take the Blood Pressure and record results in the ECCD or maternal record. | BHW | 5 minutes |
| 4. You will be given a Vitamin A supplement | 4. Checks the record and give Vitamin A to the client. Record the date in the ECCD card | PHN / PHM | 5 minutes |
| 5. Get ECCD card with corresponding Vitamin A supplementation / schedule to come back reflected on it | 5. Gives instructions as to the importance of Vitamin A and the schedule date | PHN / PHM | 5 minutes |
