Patients are issued certificate of physical fitness for sports, employment, to study, resume work or current health status (for detainees prior to their detention) or medico–legal certificate for medico– legal cases after they have been examined by the doctor in Cebu City Medical Center (CCMC). (This is applicable only in Cebu, Philippines)

2. Submit self for taking of vital signs (BP, HR, RR, Temperature) when your name is called. | 4. Takes the patient’s vital signs and record them on the patient’s chart. | OPD: Staff Nurse assigned in the different consultation cubicles. | 10 minutes |
3. Go to the respective consultation section in the OPD, Eye Clinic, Dental Clinic as your name is called or it is your turn for physical examination. | 5. Evaluates the patient’s condition by asking questions regarding patient’s health status & conducts physical examination, submits patient for laboratory work–up if necessary and determines health status of the detainee. 6. Determines fitness of patient to engage in sports, for employment, studies or resume work after having been sick. | Junior Consultant/Resident Physician | Depends on the patient’s condition/illness |
4. Request the doctor to issue medical certificate/medico legal certificate | 6.
