E102 Problem with Offline Updater not starting

Okay, so my XB1 wont load the file from the USB, it chirps then says "Something Happened" and wont start the update, or the factory reset file either. Is the issue my HDD failed, like I need a new one, or has anyone fixed this issue in the past by using an external HDD to boot the Xbox OS...?
