1. Submits letter request attached with required documents.

2. Conducts assessment/interview.

Thirty (30) minutes

3. Prepares case summary report
Social Welfare Aides

2. Make follow-up of request.
4. Processes Assistance at City Hall.
Clerk/Budget/Accounting/CTO
Fifteen (15) to Thirty (30) days
3. Get Assistance.
5. Inform client for the Release of assistance.
