1. Receives supporting documents submitted by client.

2. Conduct assessment/interview/validate with funeral home.

2. Proceed to the Office of Hon. Rodrigo Abellanosa, 2nd Legislative Building, Cebu City Hall to process referral.
3. Approves/signs the Referral Slip.

3. Submit the signed Referral Slip back to DSWS office
3. Prepares 3 copies of referral slip to be signed by Chairman of Social Services.

4. Receive copy of referral slip to be forwarded to Funeral Homes for Release of coffin.
4. Provides 1 copy of referral slip documents attached to client / applicant to be forwarded to funeral homes.
