- Two (2) recent 2 x 2 ID pictures; and
- Medical certificate from a government physician that they are physically fit and able to serve in the Palarong Pambansa.

All officiating officials must attend the Opening and Closing Programs and the Refresher Course. Those who fail to register on May 4, 2011 or those who fail to attend the Refresher Course shall be replaced by the TMA Committees following the criteria set forth in the selection of officiating officials.
All members of the TMA Committees shall observe the "No Alcohol, No Smoking and No Gambling policies of this Department during the Palaro. Violators shall be dealth with accordingly.
Transportation expenses of all members of the TMA Committees from their respective stations to JRMSU and back shall be charged to DepED OSEC Funds subject to the usual accounting and auditing rules and regulations.
